The ultimate aim of this is to help inform others and reduce the chance of it happening again.\u003C/p\u003E\n\u003Cp\u003EA learning review seeks to:\u003C/p\u003E\n\u003Cul\u003E\n\u003Cli\u003Ebe a reflective look at the event and what should or could have avoided the incident.\u003C/li\u003E\n\u003Cli\u003Eunderstand any individual\u2019s actions and the underlying reasons for those actions and decisions if they led to and/or created this incident\u003C/li\u003E\n\u003Cli\u003Emake use of relevant research/previous Learning Review recommendations to inform findings.\u003C/li\u003E\n\u003C/ul\u003E","gtmId":"","gtmClass":""},{"gridType":"RichTextBlock","content":"\u003Ch3\u003EThe review process\u003C/h3\u003E\n\u003Col\u003E\n\u003Cli\u003EIdentify why the review is necessary and what it\u2019s aims are.\u003C/li\u003E\n\u003Cli\u003EIdentify who will undertake the review and who may contribute to the review.\u003C/li\u003E\n\u003Cli\u003EAim to complete the review within two weeks.\u003C/li\u003E\n\u003Cli\u003EClarify the details of the incident, You will need to ask more than one person. Gather as much information about the incident as possible. How/when and why were key decisions made? Was POR and Guidance referred to and followed?\u003C/li\u003E\n\u003Cli\u003EIdentify the key causes \u2013 what went wrong and/or how things went wrong.\u003C/li\u003E\n\u003Cli\u003ERemember to consider the impact of the incident, on both individuals and the group. Eg: Will there be ongoing medical needs? Will funds need to be raised for new equipment?\u003C/li\u003E\n\u003Cli\u003EClarify the conclusions drawn into some key learning points. Be specific to the actual incident. Eg: Supervision \u2013 additional adults are needed for this activity. The camp kitchen \u2013 this needs to be set up with gas stoves well away from the side of the tent.\u003C/li\u003E\n\u003Cli\u003EAs part of the learning review the Group/Reviewer should identify if any further support is required for the family, other young people, staff, volunteers and the wider Scouting family.\u003C/li\u003E\n\u003Cli\u003EScout Groups may need to consider reaching out to their District/County for additional support, both in the review process and following the review.\u003C/li\u003E\n\u003C/ol\u003E","gtmId":"","gtmClass":""},{"gridType":"RichTextBlock","content":"\u003Ch3\u003ESharing learning\u003C/h3\u003E\n\u003Cp\u003EAny conclusions and lessons learned from the review will need to be shared with the local team.
